Eliminating barriers to optimal health begins with understanding the non-medical factors that can significantly influence patient outcomes. Social determinants of health (SDOH)—defined as the conditions in which people are born, grow, live, work and age—impact a wide range of health, functioning and quality-of-life outcomes and risks, according to the World Health Organization. In addition to impacting physical and mental health, these factors can have long-term effects that contribute to increased prevalence of chronic disease. In fact, SDOH can account for up to 80-90% of a patient’s overall health outcomes, according to the National Academy of Medicine.
